A 28-month-old girl, Lucy, is seen at the clinic because her grandmother, who has been the caregiver for the past year, notices a change in her behavior. She refuses to eat during mealtime and tantrums whenever she tried to use her fork. She furthermore has begun to bang her head on the floor whenever she doesn’t get her way. Grandmother informs the nurse that lately she has difficulty staying asleep throughout the night. She inquiries about using herbal remedies that she recalls her grandmother Mexico would use. She cries, “It has been too long since I had kids of my own and I do not know what to do”.
Scenario # 3:

His mother expresses a few concerns about his readiness for school since he recently turned five and there may be older children in his class. She informs the nurse practitioner (NP) that in their home they speak their primary language, which is French. She states her greatest fears is that Marcus cannot read or write any English. Upon assessment, the nurse practitioner (NP) observes Marcus to have difficulty with sound production of words beginning with the letter S. His mom inquires if it is because he lost his first front tooth and still sucks his thumb despite all efforts to get him to stop. Marcus states, “I don’t want to go to the dentist.” Before finishing the exam, his mom asks if there is anything else she can do to prepare him for school.
Scenario #4: Li, a 16-year-old boy, arrives at the clinic with his father, Mr. Chin, for a routine history and physical. He will be graduating next year and has made no plans for what he will do with his life after graduation. His dad insists he “will attend a university and make his family proud”. His family moved from China two years ago and Li has missed seeing his cousins and grandparents. During the exam, Mr. Chin steps out of the room. Li informs the doctor he feels pressured by his parents and has been depressed.
